Check Requests

  • There are rare occasions when a check request is asked for as a payment for an invoice. The check request form can be found on Financial Administration page, which is located on the University’s webpage. Click on the link to fill out this form. This form can be found on our website. Also, the information filled in on this form cannot be saved.
  • Click on the box for “Mail Check”. It can be found in the upper right corner of the form.
  • Click on the blank space next to “Name/Payee:”. Type in the vender or publisher’s name.
  • Move the cursor the mailing address lines by using the tab button on your keyboard. Social security and LSU ID number will not be filled in.
  • Click on the space next to “Requested by:” and type in the Library Director’s name.
  • Tab over to the “Department:” line and type in Acquisitions.
  • Tab over to “Date:” and type in today’s date.
  • Tab over to “Telephone Ext.:” and type in X2515.
  • There is an area that is labeled “Please Check One”. Click on the box next to “Other”.
  • Type in the following information in the “Account” section; Fund=92001, Org.=2610, Account=77005. Type in the amount the check needs to be made.
  • Type For Deposit Account in the description. This area can be found directly underneath the account section.
  • Take the check request form to the Library Director to have it signed and dated.
  • Make two extra copies of both the form and invoice/statement.
  • Proceed to pay this invoice by using the Paying Invoices procedure.
